The Florida Fire Soccer Club is a family oriented organization that is committed to helping each player learn, grow and celebrate his or her love of the game of soccer. It is dedicated to developing character, integrity and personal growth for the youth player, student-athlete and future leader. It serves the community with a continuing tradition of fun and excellence in developing players, coaches and teams.

The Florida Fire Soccer Club has grown from a one-team, one-coach organization started in 2018, to a six-team and seven-coaches soccer organization in the Pembroke Pines, Florida area uptodate 2021. It features development and skill training to boys and girls of all ages as well as the development and continueing licencing education for all of our coaches, at the Florida Fire Soccer Club we always welcome volunteer opportunities within our club or to provide community service within our city.

The Florida Fire Soccer Club is committed to developing every player to their highest potential and encourages players to compete at the highest level of their ability. Supported by an active and involved board of directors made of active Firefighters, Paramedics, Teachers and Local Business Owners as well as career Soccer Coaches. Our highly trained staff is experienced in educating and motivating players, parents, and our staff to provide the best soccer experience possible.

By electronically clicking on the “I ACCEPT” box you hereby accept to all the terms and conditions stated on this section of the registration form, if you electronically click on the “I DO NOT ACCEPT” box your participant player cannot attend to any of our practices or games or any of the activities provided by Florida Fire Soccer Club LLC or any of his affiliates.  By electronically clicking on the “I ACCEPT” box on this section of our registration form you are hereby in agreement of all and any of the terms and conditions and waivers for which you the parent/guardian/sibling/family member/other that has a personal relationship with the participant child(s) or parent/guardian/sibling/family member/other. You also state that you the parent/guardian/sibling/family member/other and your participant child(s) understand the rules  of Florida Fire Soccer Club LLC activities and programs, and hereby as the parent/guardian/sibling/family member/other that has a personal relation with the participant child(s) or parent/guardian/sibling/family member/other surrenders the right to file a lawsuit against the Florida Fire Soccer Club LLC  for any and all physical injuries, death and any other injury that will/can lead to a physical, mentally or emotionally injury that might occur to your parent/guardian/sibling/family member/other or your participant child(s) while in participation or not in participation in any and all of the Florida Fire Soccer Club LLC programs or within the limits of the facility known as FUT5IVE LLC. You also surrender the right to file a lawsuit against any and all of the Florida Fire Soccer Club LLC; Coach(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Member(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Staff(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Volunteer(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Owner(s) and their immediate or distant family members, Heir(s), friends, or acquaintances.

Fees & Refunds

Registration payments for any all activities or products for youth or adults in participation with Florida Fire Soccer Club LLC must be paid directly to the Florida Fire Soccer Club LLC program administrator. Refunds must be requested directly to the program administrator. All refunds will be placed on a pending investigation for administrator approval, refunds can be denied or granted per special circumstance only.

Age Guidelines

The Florida Fire Soccer Club LLC activities and programs are intended for children between the ages of 2 years old to 18 years old, Male and Female, with or without mental or physical limitations. With skill levels in Starter, Recreational, Beginner, Intermediate and or Advance stages.

Hours of Operation

The Florida Fire Soccer Club LLC activities and programs operate within the hours of 9:00 am to 9:00 pm from Monday to Friday. The Florida Fire Soccer Club LLC does not operate on Saturday or Sundays unless a schedule activity, program or special event has been scheduled for that particular Saturday or Sunday.

Drop-off and Pick- pick up Procedures

Participant child(s) in any of the Florida Fire Soccer Club LLC activities or programs need to arrive 15 minutes before each activities or programs begins. Parent/guardian/sibling/family member/other must park their vehicle in our parking lot space and walk their child into the activities or program and sign in the participant child(s) with a staff, volunteer, coach(s) or coach’s assistant ONLY! Participant child(s) needs to be picked up at the time for which your activities or program is scheduled to end. Parent/guardian/sibling/family member/other must sign out the participant child(s) with a staff, volunteer, coach(s) or coach’s assistant ONLY! There will be a late fee charge for participant child(s) who are picked up late after the activities or program has ended and if the coach(s) have not been notified. The late fee is $5.00 for every 5 minutes past end of activities or program. Repeated late pickups may result in the removal of the participant child(s) from the activities or program and registration and any or all fees will not be refunded or prorated for refunds. Medical Needs

 

In the event of a medical emergency, the Florida Fire Soccer Club LLC withholds the right not to engage or participate in any kind of life or death medical emergency intervention occurring to any and all participant child(s) or parent/guardian/sibling/family member/other of the Florida Fire Soccer Club LLC. The Florida Fire Soccer Club LLC Coach(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Member(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Staff(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Volunteer(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Owner(s) and their immediate or distant family members, Heir(s), friends, or acquaintances. Are not allowed to administer, introduce, help or advice with any kind of medication or drug or engage in any medical emergency treatment even if is a life or death situation to any of the program participant child(s) or parent/guardian/sibling/family member/other while participating or not in direct participation of any of the activities or programs of the Florida Fire Soccer Club LLC. All of the Florida Fire Soccer Club LLC Coach(s) Member(s) Staff(s) Volunteer(s)Owner(s) are require to (1) Immediately contact 911 in case of an emergency (2) contact the parent/guardian/sibling/family member/other of the participant child(s) in need. (3) In detail, write an incident report, a copy will be given to the parent/guardian/sibling/family member/other and another copy will be kept on file. However, BLS (Basic Life Support) or the administration of medication can be administer, introduce or help with to a participant child(s) or parent/guardian/sibling/family member/other only if a written authorization BLS waiver form has been signed and approved by the Florida Fire Soccer Club LLC along with a written note from your doctor, how to administer, introduce or help with any and all of your participant child(s) or parent/guardian/sibling/family member/other medical special condition or need. If BLS (Basic Life Support) support has been performed due to the signing of a BLS (Basic Life Support) waiver form, the parent/guardian/sibling/family member/other please be advise that the Florida Fire Soccer Club LLC will not be responsible if a participant child(s) or parent/guardian/sibling/family member/other sustains any and all physical injuries, death and any other injury that will/can lead to a physical, mentally or emotionally injury. If BLS (Basic Life Support) is provided to the participant child(s) or parent/guardian/sibling/family member/other will immediately notified the advanced life support unit (paramedics) enroute of any and all medical interventions administered to the participant child(s) or parent/guardian/sibling/family member/other at the time when the medical emergency was needed.

 

Special Circumstances

All parent/guardian/sibling/family member/other are required to inform the Florida Fire Soccer Club LLC in writing of any special circumstances which may affect the participant child(s) ability to participate fully and within the guidelines of acceptable behavior, including, but not limited to, any serious behavioral problems such autism, down syndrome, ADHD or other special circumstances regarding psychological, medical, or physical conditions. Once the notice is submitted, a conference will be scheduled with the parent/guardian/sibling/family member/other to discuss the special circumstances and whether the Florida Fire Soccer Club LLC can accommodate the circumstances of the participant child(s).

Dress Code & Code of Conduct

Any and all participant child(s) or parent/guardian/sibling/family member/other that wears inappropriate attired that displays violence, drugs, alcohol, tobacco, revealing clothing, offensive language, excessive bagginess, or any other inappropriate verbiage or images that can be offensive to any and all participant child(s) or parent/guardian/sibling/family member/other religion, culture, ethnicity, or gender within any and all of our activities or programs and facility will not be permitted. Such action can conclude in the removal of the participant child(s) or parent/guardian/sibling/family member/other from the premises.

We require all of our participant child(s) to wear black shorts, black socks, shin guards and a lightweight dry fit style T-shirt or sports-bra underneath (girls only), indoor soccer cleats or regular cleats. This is very important in order to participate in all or any of our activities or programs. While in participation of any of the Florida Fire Soccer Club LLC activities or programs, we recommend flip-flops, rubber “crocs”-style shoes, and other open-toed shoes to be used only at the end of any and all activities or programs. Not during the activities or programs.

Personal Belongings

Please put the participant child(s) name on all articles of clothing, snack bags, bags, balls, etc. Children should not wear or bring to the premises jewelry, accessories, money, mobile phone(s), tablet, game consoles, or other personal valuable possession of any kind with them to any of the programs or activities of the Florida Fire Soccer Club LLC. The Florida Fire Soccer Club LLC will not be responsible for any personal or valuable belongings of any kind that belongs to a participant child(s) or parent/guardian/sibling/family member/other.

 

Behavior Management/Discipline Policy

The Florida Fire Soccer Club LLC Coach(s), Member(s), Staff(s), Volunteer(s), Owner(s) will create a fun and safe environment for participant child(s) and their parent/guardian/sibling/family member/other in the program or activity of their choosing. Praises and positive reinforcement are used as effective methods of behavior and physical management. Participant child(s) who do not respond to these methods or who are destructive to others, learning opportunities or to the FUT5IVE LLC property will be dealt with in a professional, positive, and timely manner to correct the behavior. The following procedures will be followed in behavior management. All incident reports will be discussed privately with parent/guardian/sibling/family member/other and a copy of each report will be given to the parent/guardian/sibling/family member/other and another copy kept on file at the Florida Fire Soccer Club LLC office.

  1. In the event a participant child(s) behavior is a repeated behavior and cannot be corrected by the Florida Fire Soccer Club LLC Coach(s), Member(s), Staff(s), Volunteer(s), Owner(s) with a verbal warning or other forms of behavior modification, a first incident report will be written to document and correct behavior. A copy of the report will be given to the parent/guardian/sibling/family member/other the same day as the incident and another copy kept on file at the Florida Fire Soccer Club LLC office.
  2. A second incident report will be written if the behavior is repeated by or new behavior problems occur with the same participant child(s). This report will follow the same process as the first, a two-day suspension will accompany the participant child(s) whose behavior issues continue. Please be advised no prorated refund will be provided for suspended days. A copy of the report will be given to the parent/guardian/sibling/family member/other the same day as the incident and another copy kept on file at the Florida Fire Soccer Club LLC office.
  3. A third incident report will be completed using the same process as the first two. The Florida Fire Soccer Club LLC Coach(s), Member(s), Staff(s), Volunteer(s), Owner(s) will write this report. Coach(s), Member(s), Staff(s), Volunteer(s), Owner(s) will provide this report to the parent/guardian/sibling/family member/other. The incident report will be discussed privately with a parent/guardian/sibling/family member/other alongside the Florida Fire Soccer Club LLC Coach(s), Member(s), Staff(s), Volunteer(s), Owner(s). At this point your participant child(s) will be dismiss from any of the activities or program of the Florida Fire Soccer Club LLC.

NOTE: If an immediate dismissal occurs from any of the Florida Fire Soccer Club LLC activities or programs due to repeated offenses. Refunds will not be granted.

Insurance Consent

I understand that no insurance coverage for my participant child(s) or parent/guardian/sibling/family member/other will be granted during any and all of the activities is provided by Florida Fire Soccer Club LLC activities/practices/games/other.

Informed Consent

By registering for any or all of the Florida Fire Soccer Club LLC activities or programs, I understand and agree that if a portion of the program is unable to be completed due to unforeseen circumstances under the responsibility of Florida Fire Soccer Club LLC, I will receive a prorated credit on my account for the uncompleted portion of the program.

Marketing & Promotional Material Waivers and Consent

By signing this section, I, as the parent/guarding/sibling/family member/other of my participant child(s) grant permission to the Florida Fire Soccer Club LLC to use pictures of me the parent/guarding/sibling/family member/other and of my participant child(s) in promotional literature, print marketing material, social media marketing, videos and other promotional or marketing literature. For the use on the Broward Futsal & Indoor Soccer League Corp, Grand Palms Community, Amp Fitness, FUT5IVE LLC, Landsharks FC Academy, JGA Creative Group, JGA Creative Cloud, JGA Printing, JGA Sports Management Group, MSA Academy LLC, Florida Fire Soccer Club LLC website. I understand my participant child(s) or parent/guarding/sibling/family member/ other names (s) will not be published or compensated.

Participation & Facility Waiver

I the parent/guardian/sibling/family member/other of (participant child(s), hereby assume all the risks and hazards incidental to the conduct of the activities at the  Broward Futsal & Indoor Soccer League Corp, Grand Palms Community, Amp Fitness, FUT5IVE LLC, Landsharks FC Academy, JGA Creative Group, JGA Creative Cloud, JGA Printing, JGA Sports Management Group, MSA Academy LLC, Florida Fire Soccer Club LLC facility, organization, transportation from and to any of the activities/practices/games/other. My participant child(s) is fit for the program(s) in which I have enrolled him/her. I hereby releasees and shall not defend indemnify and hold harmless releasees from any claim and any liability that I the parent/guardian/sibling/family member/other or my participant child(s) may allege against releasees (including any and all legal fees and costs) as a direct or indirect result of injury or death, mental injury, physical injury, psychological injury, or emotional injury to me the parent/guardian/sibling/family member/other or my participant child(s) while in participation or not in participation in any of the Broward Futsal & Indoor Soccer League Corp, Grand Palms Community, Amp Fitness, FUT5IVE LLC, Landsharks FC Academy, JGA Creative Group, JGA Creative Cloud, JGA Printing, JGA Sports Management Group, MSA Academy LLC, Florida Fire Soccer Club LLC facility, organization, transportation from and to any of the activities/practices/games/other. Even if it was caused by any or all of the Broward Futsal & Indoor Soccer League Corp, Grand Palms Community, Amp Fitness, FUT5IVE LLC, Landsharks FC Academy, JGA Creative Group, JGA Creative Cloud, JGA Printing, JGA Sports Management Group, MSA Academy LLC, Florida Fire Soccer Club LLC facility, Coach(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Member(s) and their immediate or distant family members, Heir(s), friends, or  acquaintances, Staff(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Volunteer(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Owner(s) and their immediate or distant family members, Heir(s), friends, or acquaintances negligence or wrongful actions. I relinquished the right to all releasees or others to the maximum extent permitted by law in the state of Florida. I will give up any and all right to pursue in a legal lawsuit and releasees on my behalf parent/guardian/sibling/family member/other or on behalf of my participant child(s) regarding claim arising from related to my participant child(s) participation or not in participation in any of the Broward Futsal & Indoor Soccer League Corp, Grand Palms Community, Amp Fitness, FUT5IVE LLC, Landsharks FC Academy, JGA Creative Group, JGA Creative Cloud, JGA Printing, JGA Sports Management Group, MSA Academy LLC, Florida Fire Soccer Club LLC facility, organization, transportation from and to any of the activities/practices/games/other. I acknowledge that, by signing this document, I am releasing the Broward Futsal & Indoor Soccer League Corp, Grand Palms Community, Amp Fitness, FUT5IVE LLC, Landsharks FC Academy, JGA Creative Group, JGA Creative Cloud, JGA Printing, JGA Sports Management Group, MSA Academy LLC, Florida Fire Soccer Club LLC as well as their Coach(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Member(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Staff(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Volunteer(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Owner(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Representative(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Agent(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Promoter(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Sponsor(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, Affiliate(s) and their immediate or distant family members, Heir(s), friends, or acquaintances, (Collectively “Releasees”) from any and all liability direct, indirect or by negligence in result of injury or death, mental injury, physical injury, psychological injury, or emotional injury to me the

I have read this releasees carefully before signing. I understand what releasees means and what I am agreeing to by signing this document.
